The public inquiry is England's first into mental health deaths, with evidence being heard in London after sessions in September and November.

Ms Coles accused health trusts across the country of failing in their duty of care to patients.A 46-year-old man was arrested on suspicion of attempted murder but has since been released on bail, while a second man in his 40s was arrested on Tuesday on suspicion of causing grievous bodily harm.

Detective Inspector Mark Davidson thanked the public for their "patience and co-operation over the last couple of days while we've been carrying out our enquiries".He added while the investigation is at an early stage, the force were "not looking for anyone else in connection with the incident and there's no threat to the wider community".
